Post-Hospitalization Program
New England Pediatric Care is pleased to announce a new Short-Term Post-Hospitalization Admission Program for children who are about to be discharged from the hospital but need interim skilled medical care to maximize their recovery period. This program offers transitional care for children who are hospitalized as a result of major surgeries or complicated medical conditions, but will eventually return to their home or to a residential placement.
N.E.P.C. will conduct its own assessment to ensure it can meet the needs of the short term patient. With a good continuum of care and family teaching, short term skilled stays at N.E.P.C. can reduce the likelihood of re-admission to the hospital and help to ensure the child’s success at home or school.
Families are encouraged to visit their children whenever they wish. They continue to be the sole decision makers regarding any medical or educational plans developed for their children and can be involved with the daily activities of their child to whatever extent they wish. Parents can have lunch, attend a medical appointment or therapy session, or drop in to say good night, for example.
The new admission program allows for a 30-day stay, with extensions possible up to 90 days. It was approved in January 2009 by The Executive Office of Health and Human Services and the Department of Public Health, Bureau of Family Health and Nutrition Medical Review Team (MRT).
The N.E.P.C. staff will work directly with referring hospitals, families and providers to facilitate short term admission to N.E.P.C. and ensure that children return to their homes or community placements as quickly as possible.
